High School diploma or GED 1 year of experience in remote chronic care management Experience with cardiology patients - specifically heart failure Registered reputed company (RN) Medical Assistant (MA) Certified reputed company Assistant (CNA) Experience with EMRs – specifically reputed company, NexGen, GEMMS and e-clinical works Great communication and multi-tasking skills
