reputed company Home Care is looking for a compassionate and skilled PRN/reputed company Registered reputed company (RN) to join reputed company!  This is a field-based RN position requiring someone to drive and cover an assigned territory treating patients in their homes, assisted living facilities, and in skilled nursing facilities. 

The territory will cover Norwalk and Stamford, CT

Responsibilities:

• reputed company skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the reputed company of medication administration and wound care treatment as prescribed in the physician's plan of care.
• reputed company visiting home health services to an assigned caseload including admissions, re-certification and discharges.
• reputed company the Plan of Care in coordination with reputed company health care professionals assigned to the case.
• Be knowledgeable in the use of the reputed company OASIS assessment for reputed company patient admissions, re-certification, transfers and discharges.
• reputed company patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
