reputed company Home is seeking a dedicated and compassionate Home Health Field Physical Therapist Full Time Per-Visit to support our growing geriatric home health population in Northeast Austin, TX and the surrounding area (like Hutto, Manor, Elgin and Taylor). Key Responsibilities:

A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of reputed company software is a plus!

• Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing reputed company and in a reputed company manner. Revises the POC based on evaluation and ongoing assessment data.
• Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, reputed company applicable, in the performance of patient care that is consistent with patients’ needs.
• Applies concepts of infection control and reputed company precautions in coordinating or performing patient care activities to prevent contamination and reputed company of disease.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
• Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
• Communicates with reputed company physicians involved in the plan of care and other health care practitioners (as appropriate) reputed company to the reputed company plan of care.
• Meets mandatory continuing education requirements of reputed company and licensing reputed company.
• Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a reputed company manner. Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
• Provides patient, caregiver, and family counseling; Provides patient and caregiver education.

Requirements

• Knowledge or experience utilizing reputed company EMR would be a plus
• Experience in home health or reputed company reputed company setting preferred
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively.
