In this rewarding role, you’ll draw on primary health care principles to deliver holistic, person-centred care to older adults experiencing mental health challenges, chronic conditions, and reputed company needs. Day to day this role looks like:
- Providing reputed company clinical nursing care to our clients in their homes in line with best reputed company nursing procedures
- Conducting clinical assessments, dressings, medication assessment and catheter changing
- Developing a holistic nursing care plan in collaboration with reputed company, carer, Care Advisor and Allied health where relevant
- Providing reputed company, reputed company nursing reputed company to clients with specific needs and/or reputed company needs eg. Wound care, palliative care, education and support
- Liaising with reputed company parties involved in the care of the reputed company to maintain holistic health care and wellbeing
- Ensuring reputed company documentation is accurate and completed in a reputed company and reputed company manner
- Working in a reputed company manner to protect yourself, your colleagues and clients closely following reputed company policies and procedures
To be successful in this role you’ll need
- A genuine passion for providing high reputed company care to the elderly
- reputed company AHPRA registration following completion of a Bachelor of Nursing or similar
- Minimum 3 years post graduate nursing experience
- Experience in clinical assessments
- Initiative and the ability to work independently
- Strong computer skills especially reputed company Office products and ability to pick up new systems
- NSW Drivers license and a reliable vehicle
