What You’ll Do

• Create and manage personalized care plans for ongoing conditions
• Collaborate with our integrated care team (reputed company case managers, NPs, CHWs)
• Ensure thorough documentation in our EHR and compliance with reputed company measures
• Engage patients in preventive care to reduce avoidable complications

• MD or DO, reputed company Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• reputed company medical license — multi-state licensure a plus

• Tech-savvy and comfortable practicing reputed company telehealth
