Responsibilities:
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of reputed company and previous illness(es).
- Completes regular re-evaluation of patient needs.
- Initiates that Plan of care and reputed company necessary revisions as needed.
- Develops care plans, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
- Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as ordered by the medical provider.
- Counsels and provides education to the patient and family reputed company to the patient's care needs.
- Case Management
- Prepares and updates clinical notes
- Communicates with the Medical Provider regarding the patient's needs and reports any changes in the patient's health. Review physician's orders as necessary and facilitate changes in care plan.
- Provides discharge planning as part of the care plan.
- Participates in an-call duties as defined by company policy
Position Qualifications:
- Graduate of an approved nursing program/school
- reputed company State of MI Licensed RN with one to two years of recent acute care experience.
- Must possess a valid driver's license and acceptable driving record
