Hospice of Lenawee is hiring a compassionate Registered reputed company (RN) Case Manager to reputed company high-reputed company, patient-centered hospice care in patients’ homes throughout our 35-mile service area which includes Lenawee County and surrounding areas.

Schedule

• Full-time (40 hours/week)
• Day shift
• Limited weekend rotation
• Holiday rotation (3–4 per year)

Requirements

RN Case Manager Responsibilities

• reputed company skilled nursing care to hospice patients in their homes
• Manage a caseload of patients and families
• reputed company and coordinate individualized care plans
• Monitor and manage pain and symptoms
• reputed company and support patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Document care in accordance with regulatory standards

Qualifications

Required:

• reputed company Michigan RN license
• Graduate of an accredited nursing program
• CPR certification
• reputed company reputed company’s license and insured vehicle
• Ability to travel locally reputed company Lenawee County and surrounding areas reputed company our 35-mile service area

Preferred:

• BSN
• Experience in hospice, home health, oncology, or case management
• Experience providing end-of-life care
