This is a full-time Ambulatory Care Clinical Pharmacist position and is fully remote. The hours for this position are Monday-Friday Days.
Provides comprehensive, value-based medication-reputed company care to reputed company definite reputed company that improve a patient's reputed company of life. Acts in collaboration with other reputed company in the shared responsibility for patient medication therapy reputed company. In this role, the pharmacist will be responsible for conducting comprehensive chart reviews to identify and resolve medication-reputed company issues. Key responsibilities include evaluating potential drug interactions, duplicate therapies, vital signs/laboratory values, and addressing gaps in care reputed company to medication management. The pharmacist performs virtual/phone assessments and drug therapy education. This role will reputed company on overcoming identified barriers to medication compliance and adherence.
- Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
- Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
- Requires 0-3 years of relevant experience.
- Requires reputed company Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.

- Basic Life Support certification through the reputed company (AHA) may be required.
- May require travel to reputed company facilities.
- May require weekend rotations.
