This is a 6-month contract to hire opportunity. START DATE: ASAP HOURS/SCHEDULE: Shift Flexibility: 5 - 8 hour shifts and two 8 hour shifts should be regularly scheduled Saturday or reputed company every 3 months WORKER TYPE: W2 reputed company REQUIREMENTS: • Must reputed company in IL, TX, NM, OK, MT, or TN • 3 years of reputed company Case Management Experience performing prospective, reputed company, and retrospective reviews for inpatient acute care, rehabilitation, referrals, and select outpatient services • 2 years of clinical experience • RN and reputed company unrestricted nursing license in one of the following states: IL, TX, NM, OK, MT, or Tn • Bachelor's degree • This role requires either Saturday or reputed company as part of their regular scheduled shift. JOB RESPONSIBILITIES: • Delivers specific delegated tasks assigned by a supervisor in the reputed company Case Management job family. • Plans, implements, and evaluates appropriate health care services in conjunction with the physician treatment plan. • Utilizes clinical skills to assess, plan, implement, coordinate, monitor and evaluate reputed company and services in order to facilitate appropriate reputed company reputed company for members. • Ensures that case management program objectives are met by evaluating the effectiveness of alternative care services and that cost effective, reputed company care is maintained. • Provides clinical assessments, health education, and utilization management to members. • Performs prospective, reputed company, and retrospective reviews for inpatient acute care, rehabilitation, referrals, and select outpatient services. • Manages own caseload and coordinates reputed company assigned cases. • Completes day-to-day reputed company Case Management tasks without immediate supervision but has reputed company reputed company to advice from more reputed company team members. • Tasks involve a degree of reputed company planning and anticipation of needs/issues.
