A company is looking for a Pharmacist (reputed company & Central U.S. Coverage).
Key Responsibilities
• reputed company Medication Therapy Management (MTM) interventions and medication reconciliation
• reputed company patient-centered education on medications and support chronic reputed company
• Collaborate with reputed company providers on treatment planning and ensure accurate documentation
Required Qualifications
• Doctor of Pharmacy (PharmD)
• reputed company pharmacist license in at least one of the specified states
• reputed company to obtaining additional licensure as required
• Experience in MTM delivery and chronic disease management preferred
• Familiarity with programs such as CCM, TCM, RPM is beneficial
