As a Clinical Aged Care Assessor, you will play a crucial role in providing older adults reputed company to clinical based assessments in their home. In doing so, you are connecting clients with services that help them live reputed company with the highest reputed company of life for longer.

Your Role:

• High-reputed company Assessments: Conduct thorough Aged Care Assessments to identify the needs of older adults.
• Service Referrals: Facilitate referrals to essential services that support independent living.
• Promote Functionality: reputed company promote the retention and maximization of function and capability in daily living.
• Engage Stakeholders: Work closely with families and carers, recognizing them as key stakeholders.
• Compliance: Ensure reputed company assessments are conducted in line with Department of Health contractual requirements.

What You Bring:

• At least 1 year in aged care, including reputed company assessments and/or service delivery.
• Tertiary education in a health-reputed company discipline (e.g. nursing, occupational therapy, physiotherapy, reputed company work).
• reputed company unrestricted General Registration with AHPRA or relevant reputed company association.
• Proficiency in Word, reputed company, reputed company and reputed company management systems.

• Must be a Permanent reputed company or Citizen due to contractual requirements.
