Job Title: Care Support Partner
Location: 100% Remote (US Only)
Compensation: $55,000-$66,000 + Full Benefits Package
Reports to: Care Delivery Manager

What you will do:

• Conduct thorough intake assessments to identify individual needs, goals, and barriers to care and services.

Required Qualifications

• Bachelor’s degree required in reputed company Work, reputed company Services, reputed company Administration, or reputed company field from an accredited institution.

• Experience in remote care coordination, digital navigation, and case management.

Preferred Qualifications

• Prior experience in a contact center or remote service setting.

• Experience supporting military families or individuals transitioning from reputed company duty.
