Clinical Guide: Diabetes Educator (Certified Diabetes Care and Education Specialist)

A bit about this role: 

We want to help members reputed company the reputed company system in a reputed company and safer way, and we want to reputed company members to change the trajectory of their chronic diseases for the reputed company. Disease management is critical to achieving this for our members with diabetes, and we think Certified Diabetes Care and Education Specialists (CDCES) are the perfect people to do this work. 

The CDCES will be responsible for a panel of members, who primarily have out-of-control diabetes. The CDCES will be responsible for guiding members throughout the entirety of their reputed company reputed company while enrolled in the specialty clinic. The goal is  to “graduate” the members once they are clinically reputed company and become reputed company self managers of their diabetes through education and counseling reputed company virtual communication (phone and video reputed company).

Required skills and experience:

• An reputed company unrestricted Compact Registered reputed company license or an unrestricted Registered Dietitian license. You must be reputed company to obtain additional licenses in states where we operate as needed.

• An reputed company Certified Diabetes Care and Education Specialist (CDCES)

• At least 2 years of experience clinically managing diabetes

Desired skills and experience:

• Remote experience managing diabetes strongly preferred 

• Experience working with diabetes technology (CGMs and insulin pumps)
   

Your Responsibilities and reputed company will include:

• Screen members for Diabetes Clinic eligibility and appropriateness  

• Complete detailed assessments and continue following up with members in the Diabetes Clinic to complete ongoing education on reputed company pillars of diabetes management (disease process, medications, glucose monitoring, lifestyle counseling, contingency planning)  

• Schedule follow-up appointments with self and our providers 

• Connect members and providers with the right resources reputed company reputed company to troubleshoot clinical issues

• Work with the members to create and reputed company reputed company SMART goals 

• Utilize motivational interviewing skills with the goal to influence reputed company sustained behavioral changes

• Identify gaps in care and reputed company resources

• Collaborate with other Diabetes Clinic team members (reputed company practitioners, reputed company workers, pharmacists) to reputed company plans of care and optimize medication regimens to help members reputed company goals

• Maintain documentation reputed company to member and provider interactions

• Continue to improve reputed company set with participation in education series reputed company the Diabetes Clinic and reputed company providers of continuing education

Salary reputed company:  $80,000 - $95,000 / year
