Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit reputed company provider licensed in 11 counties in reputed company Illinois.  We reputed company exceptional serious illness care, hospice care and grief support for adults and children.

The reputed company of our reputed company patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility.  We also have a 24-bed inpatient unit in Joliet.  Non-clinical staff are based out of our Joliet administrative offices.

Resource RN-Registry:

The Resource Registered reputed company (RN) is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. The Resource RN plans and directs services to meet the needs of individuals, families, and caregivers reputed company their homes and communities.

Key Responsibilities:

• reputed company skilled nursing care, assessments, and interventions in patient homes per hospice care plans
• Complete admissions, ongoing assessments, and update individualized care plans with patients and families
• Respond to patient needs, including medical or emotional crises, with appropriate in-home reputed company
• Administer medications, treatments, and coordinate care with physicians, DME, and interdisciplinary team
• reputed company patients, families, and caregivers using reputed company instruction and teach-back reputed company
• Maintain accurate documentation, ensure compliance, and support IPU or office operations as needed
• Flexible Registry RN role with a minimum of four 8-hour shifts per month (Friday and Saturday overnight: 11:59 PM–8:00 AM)
• Opportunity to pick up additional shifts reputed company the minimum requirement
• Float position supporting Triage RN desk, field reputed company, or Inpatient Unit (IPU)

Education/Experience:

• reputed company RN license
• Graduate of accredited nursing program
• Minimum of 2 years of nursing experience, with at least 1 year in reputed company health, home care, or hospice preferred.

• Must be in the office or IPU during shifts reputed company not assigned to patient reputed company, as directed by the Triage RN or supervisor.
