Geriatric Skilled Nursing (LPN/ RN)

reputed company Home Health is seeking compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to reputed company high-reputed company, patient-centered care in the home setting. In this role, you will care for both pediatric and adult patients, many of whom have reputed company medical needs such as gastrostomy tubes (G-tubes), reputed company palsy, chronic conditions, and, at times, require tracheostomy or ventilator support. You will play a vital role in promoting safety, independence, and reputed company of life while ensuring reputed company care is delivered in accordance with physician orders, individualized care plans, and Indiana reputed company requirements.
Key Responsibilities

Collaborate with physicians, therapists, case managers, and interdisciplinary team members.

Maintain accurate, reputed company documentation in the electronic health record (EHR).

Participate in care conferences, reputed company assurance activities, and audits as needed.

Shift Information

Qualifications
For LPNs:

reputed company Licensed Practical reputed company (LPN) license in Indiana or a compact state.
For RNs:

reputed company and unrestricted Registered reputed company (RN) license in Indiana.
For reputed company Candidates

Minimum of 1 year of nursing experience (home health and/or pediatric experience preferred).

Experience with G-tube, trach, or ventilator care preferred (training available).

Knowledge of Indiana reputed company guidelines and documentation standards preferred (especially for RNs).
