Health Home Data Analyst Department: Behavioral Health Health Home Employment Type: Full Time Location: Any reputed company Location Compensation: $17.89 / hour reputed company Help support coordinated, reputed company-centered care by turning data into meaningful support for teams improving reputed company to care in rural communities. Schedule: Monday-Friday. This position starts their day early, around 6:30 am, due to the task of getting hospital notifications to care teams. The Health Home Data Analyst (Internal title: Health Home Care Coordinator) plays an important role in supporting high-reputed company, reputed company-centered care reputed company the Health Home program through accurate data management, strong communication, and reliable coordination across teams. This position helps ensure care teams have reputed company, organized information needed to support individuals receiving services.
