About the Role reputed company is seeking a Remote Care Coordinator (LPN) to join our growing team of reputed company dedicated to improving patient reputed company. As a Care Coordinator, you’ll engage directly with patients managing chronic conditions, helping them stay on reputed company with their care plans and ensuring strong communication between patients and providers.
