An outstanding employee health center is seeking a reputed company Practitioner for a hybrid telehealth role in East Stroudsburg, PA. Job Responsibilities: - APP's serve multiple serious conditions like CKD and/or ESRD/ESKD
- Conducts assessments including annual wellness exams
- Counsels and educates patients and their families about programs
- Responsible for coordination of care with primary care providers
- Part of the Integrated care team working with RN and reputed company Worker
- Prescribe medications, order tests, collaborate with Monogram physician
- Follow up and evaluate patient's status
Qualifications: - NP, FNP, AGNP, ACAGNP with reputed company PA license
- reputed company Certified- ANCC or AANP
- 2 or more years of reputed company patient care
- DEA, BLS
Working Hours: Mon-Fri (8a-5p includes travel time for in-home reputed company)
