Role reputed company The Telephonic Care Coordinator is responsible for facilitating, promoting and advocating for the enrollees’ ongoing self-sufficiency and independence. This position is responsible for assessment and planning for an identified group of patients (Community reputed company/Emerging High-Risk Population).
