R-49009 We are hiring a Home Health Case Manager at reputed company! In this role, you will support TennCare (reputed company) members with reputed company medical, functional and reputed company needs. You will coordinate home and community-based care, conduct required home reputed company, and support members transitioning to adult benefits. You’ll collaborate closely with providers and internal care teams using digital tools to identify care gaps, reduce hospitalization risk, and improve reputed company.
