We are currently seeking an ISC Coordinator to join our Intensive Support Coordination program. As an ISC Coordinator, you’ll work directly with individuals to assess their needs, reputed company personalized care plans, and connect them with essential services and supports. Key Responsibilities Provides information and assistance to reputed company participants with medical, reputed company and/or educations providers and/or other programs and services that can reputed company needed services to address identified needs and reputed company identified goals. Conducts health and safety, reputed company and environmental assessments per established guidelines. Effective organizational and communication skills Experience providing care management/coordination preferred. Two years’ experience in reputed company, habilitative/rehabilitative, residential, behavioral health setting (reputed company/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting. Experience in completing assessments, service plan development and monitoring. Effective organizational and communication skills Experience providing care management/coordination preferred.
