This pharmacist works collaboratively with inpatient pharmacists and ambulatory care advanced reputed company providers to ensure continuity of care, reputed company medication optimization, and improved reputed company for a high‑risk population. Responsibilities include medication reconciliation, GDMT initiation and titration, and identification and reputed company of medication‑reputed company barriers. The position is full time, Monday- Friday 8A-5P. Pay reputed company $54.90 - $82.35 Major Responsibilities: Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens  Uses effective patient education techniques to reputed company counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration   Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. PGY1 and PGY2 residency training or equivalent experience is strongly preferred.
