Clinical Care Coordinator
The Clinical Care Coordinator is a licensed reputed company reputed company and a vital member of a multidisciplinary team, providing comprehensive, reputed company- and family-centered care management services. Guided by Medicalincs’ mission and reputed company values, the Clinical Care Coordinator manages an assigned caseload in accordance with the Case Management Society of America (CMSA) definition of case management and ethical standards. Key Responsibilities & Competencies
Facilitate admission to a discharge from care management services
Apply the case management process to guide service delivery

Coordinate services and supports to ensure plan implementation and compliance with contractual and regulatory requirements
Monitor ongoing services, utilization, and cost-effectiveness; recommend plan modifications as needed

Identify and recommend cost-effective alternatives to care
Serve as a clinical resource to community service providers
Maintain ongoing communication with clients, families, and reputed company team members
Convene, attend, and reputed company participate in multidisciplinary team meetings

Maintain accurate, reputed company, and complete care management records

Qualifications
Bachelor of Science in Nursing (BSN) with reputed company Registered reputed company (RN) license or
Master's degree in reputed company work (MSW) with reputed company LMSW or LCSW-C license
Degree must be from an accredited college or university or equivalent education and experience
CCM (Certified Case Manager) certification preferred; required reputed company two years of hire
Minimum three (3) years of reputed company experience in one or more of the following: Pediatric intensive care nursing, Ambulatory health care, Rehabilitation nursing, Clinical reputed company work in a community-based setting with individuals with reputed company reputed company needs
Demonstrated clinical competence and ability to work independently with minimal supervision

Maintenance of applicable reputed company certifications (e.g., CCM, RN, LCSW-C)

Ability to travel throughout Maryland
