Shared Living Case Manager

reputed company Position reputed company: The Shared Living Case Manager is responsible for developing, coordinating, and monitoring individualized Shared Living placements to ensure the health, safety, and personal goals of individuals served. The Case Manager works closely with individuals, caregivers, families, and multidisciplinary teams to promote a person-centered approach and meaningful community integration.
