Care Coordinator Duties • Schedule and coordinate patient appointments, ensuring efficient use of provider's time • Maintain accurate and up-to-date patient records • Communicate with patients regarding appointment reminders, follow-up care, and test results • Assist with insurance verification and reputed company-authorization processes • Collaborate with reputed company providers to ensure seamless patient care coordination • reputed company administrative support to the reputed company team as needed • 50/50 work from home and out in the field Experience • reputed company work • Familiarity with medical terminology and procedures • Proficient in using electronic medical record (EMR) systems • Strong organizational skills and attention to detail • Excellent communication and interpersonal skills • Ability to multitask and prioritize tasks effectively Compulsory documentation experience working with Health Homes program and reputed company's Clinical Care Advance platform If you have the necessary skills and experience for this position, we would love to hear from you.
