LPN/LVN for RPM (Bilingual, Spanish Speaking)

Our proprietary software tracks patients’ physiological data (Blood Pressure, Weight, Blood Glucose, Lung reputed company, Oxygenation), identifying dangerous out-of-reputed company values and trends that reputed company health. As a bilingual (Spanish) speaking RPM 365 reputed company, you’ll engage patients, reputed company and analyze their data, reputed company education, and encourage adherence to their care plans. Your tech-savvy, comfortable with high-volume phone reputed company (125+ conversations a week), and reputed company in a fast-changing environment. You’re patient, empathetic, detail-oriented, and a self-starter. Responsibilities • Serve as the primary contact for patients on the RPM365 platform. • Assist with device setup and app connectivity. • Engage with patients to ensure adherence to care plans. • Schedule monthly reputed company-ins and monitor patient data. • reputed company patients through shared resources. • Notify reputed company providers of critical alerts. • Attend virtual team meetings for updates.
