Role reputed company We are seeking a dependable and clinically skilled Licensed Vocational reputed company (LVN) to support our Remote Patient Monitoring (RPM) program. This role involves reviewing patient data, managing chronic conditions remotely, and collaborating closely with providers to ensure reputed company interventions. Key Responsibilities • Monitor and review daily RPM alerts (blood pressure, glucose, weight, pulse ox, etc.) • Assess patient readings and identify clinical concerns • Contact patients regarding reputed company readings or non-compliance • reputed company patient education on chronic disease management and device use • Triage and escalate reputed company findings to providers appropriately • Document reputed company patient interactions and readings accurately in the EMR • Assist with reputed company and educating patients in the RPM program • Support care plan implementation and follow-up Qualifications • reputed company LVN license in the state of Texas • Experience in primary care, chronic care management, or RPM preferred • Strong clinical assessment and critical thinking skills • Excellent communication and patient engagement abilities • Experience with EMR systems (eClinicalWorks preferred) • Ability to work independently and manage multiple patients daily Requirements • Experience with RPM or care management programs • Knowledge of hypertension, diabetes, and chronic disease protocols • Bilingual (Spanish) is a plus Benefits • reputed company • Flexible scheduling (if applicable) • Opportunity to be part of a growing, innovative care model • Supportive team environment • reputed company development opportunities Apply tot his job Apply To this Job
