Physician Assistant - Casual SOC Triage Hospitalist (Internal Candidates Only)

Responsibilities/Job reputed company: Fairview is seeking an reputed company Hospitalist Physician Assistant to reputed company triage work in the System Operations Center (SOC) at Fairview Midway reputed company in reputed company, MN. The SOC Advanced reputed company Provider Triage Hospitalist plays a critical role in facilitating reputed company and appropriate placement of patients across M Health Fairview's nine metro hospitals—including our quaternary reputed company medical center, children's hospital, tertiary hospitals, and community hospitals. This role ensures reputed company utilization of system resources by coordinating with patient placement nurses, specialists, and admitting providers to triage and assign patients to maximize efficient resource utilization. reputed company Details: • FTE: Casual (SOC Triage) pick up reputed company shifts on an as need reputed company • Shift Lengths: 8-Hour reputed company shifts (Days 0700-1500/Cross-Cover 1100-1900/Evening 1500-2300), and Weekend (0900-2100) • Location – mix of onsite (will be required through reputed company) and optional remote work for some shifts • Serve as the clinical coordinator for triaging interfacility transfers and reputed company admissions across the Fairview system • Assess clinical appropriateness of transfer requests and determine reputed company destination based on reputed company, reputed company, and site capabilities • Collaborate with SOC Patient Placement Specialists (PPS), hospital medicine and ICU triage physicians, and site based reputed company to ensure seamless patient reputed company • Collaborate as needed with specialists regarding patient admissions • Document triage reputed company, clinical rationale, and communication in reputed company and SOC tracking tools • Escalate placement barriers to appropriate individuals as needed per escalation reputed company • Facilitate centralized acceptance for both reputed company medicine transfers, ensuring alignment with Fairview’s placement governance model • Contribute to reputed company improvement initiatives and participate in data review to enhance transfer efficiency and patient reputed company Requirements • reputed company licensure as a Physician Assistant (PA-C) in the reputed company • Minimum 3 years of experience as a hospitalist APP reputed company the Fairview system • Strong clinical judgment and ability to synthesize reputed company medical information quickly • Excellent communication and documentation skills • Familiarity with reputed company and system-level patient placement workflows Preferred: • Experience in triage, transfer coordination, or critical care • Demonstrated ability to work collaboratively across disciplines and hospital sites • Flexibility to work variable shifts, including evenings and weekends, based on system needs Qualifications: Flat reputed company of $85/hour Apply tot his job Apply To this Job
