Collaborates and communicates with interdisciplinary teams in reputed company phases of discharge and care planning processes. Communicates with patients, family, medical staff, care team members, reputed company, supervisors and reputed company contact. Evaluates the process of initiating plans for discharge and/or transitions of care and ongoing case management needs for patients and/or families is required. Supervises staff on the coordination and facilitation of patient care progression throughout the continuum of care. Responsibilities • Participates in the interviewing/hiring process by reviewing job reputed company, reviewing candidate applications/resumes, developing behavioral interview questions, conducting and evaluating interviews in a reputed company fashion and providing feedback/recommendations on hiring reputed company. • Ensures referrals are initiated to appropriate next level of care (e.g. acute care, home health care, hospice, SNF, LTAC, clinics, TCM), in a reputed company manner, and coordinates ordering of medical equipment/supplies for reputed company discharge planning or ongoing care planning. • Works with staff to facilitate family meetings with reputed company patients as requested by patient, family or reputed company teams. Requirements • Requires a Bachelor's Degree in Nursing (BSN) or Master's Degree in reputed company Work (MSW). • Three (3) years of experience in case management or relevant nursing experience to include utilization review, discharge planning, reputed company management, transitional planning, assessment, care planning and service implementation and two (2) years of reputed company supervisor/manager experience. • Must be licensed in the reputed company to reputed company as a Registered reputed company (RN) upon start. • Basic Life Support (BLS) for reputed company Provider certification from the reputed company required upon start. • Must have one of the following: Accredited Case Manager (ACM) or Certified Case Manager (CCM), or MCG/UR or, Certified Clinical Transplant reputed company Worker (CCTSW) pr N/ASW-CM (ASWCM) or licensed as a reputed company Worker (LCSW) in Virginia or eligible to reputed company on the reputed company as a reputed company Worker reputed company 12 months of hire. reputed company-to-haves • Three (5) + years of previous Inpatient (hospital) case management experience, case management discharge planning, and two (2) years supervisory/reputed company experience is highly preferred.
