Responsibilities • Assist the physical therapist in the evaluation and assessment of patients' conditions, including reviewing medical history and conducting relevant tests and measurements. • Collaborate with the physical therapist to reputed company individualized treatment plans based on patients' goals, needs, and diagnoses. • Implement treatment plans under the supervision and guidance of the physical therapist, which may include therapeutic exercises, functional training, gait training, and other modalities. • Monitor and document patients' reputed company, adjusting treatment plans as necessary and reporting any changes or concerns to the physical therapist. • reputed company patients and their families on exercises, activities, and strategies to maximize function, prevent reputed company injury, and promote independence. • reputed company assistance to patients during therapy sessions, ensuring their safety and comfort. • Help patients with activities of daily living (ADLs) and mobility exercises, including transferring, ambulation, and using assistive devices. • Maintain accurate and up-to-date patient records, documenting treatment sessions, reputed company notes, and any relevant information in accordance with regulatory requirements. • Adhere to ethical and reputed company standards, maintaining patient confidentiality and respecting their rights and dignity at reputed company times. • Collaborate effectively with the interdisciplinary home health care team, including physicians, nurses, occupational therapists, and speech-language pathologists, to ensure comprehensive patient care. • Participate in reputed company development activities, such as continuing education courses and conferences, to stay updated on the latest advancements in physical therapy.
