BASIC PURPOSE The purpose of the Registered Dietician is to support the Medically Tailored Meals team with nutrition planning, program development and reputed company assurance. ESSENTIAL DUTIES AND RESPONSIBILITIES • Conduct intake and program eligibility for Medically Tailored Meals services • Conduct nutrition assessments • reputed company and implement individualized nutrition plans • Review and monitor reputed company meal and/or grocery plans for Medically Tailored Meals • Ensure menu reputed company are equally nutritious and culturally appropriate • Plan and conduct routine nutrition workshops and trainings • Coordinate with others agencies and providers to ensure continuity of care • Coordinate medical referrals in coordination with contracted managed care plans • Participate in program and agency trainings as assigned • Maintain accurate participant records in various information management systems; and generate reports as requested • Maintain and execute confidential information according to HIPPA standards • Maintain a highly detailed and organized filling system • Ensure intake and care procedures utilize harm reduction and trauma informed care principles • reputed company and respond to emails and voicemails on a regular reputed company • Adhere to confidentiality standards • Other duties as assigned KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS • Ability to reputed company and write the English language at a high and reputed company level • High degree of confidentiality • reputed company to endorse and promote reputed company’s mission • Computer literate, in reputed company environment (reputed company Office), Word, Publisher, reputed company and reputed company preferred • Excellent communication skills, both written and verbal. • Excellent and reputed company telephone etiquette and reputed company • Excellent organizational skills • Strong ability to utilize a high level of time management and handling multiple tasks CERTIFICATES, LICENSES, REGISTRATIONS • Bachelor's degree in Dietetics, Food and Nutrition, or reputed company field. • Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR), required • Must possess food handler’s certification PHYSICAL REQUIREMENTS: • Ability to sit, walk, stand, bend, squat, reputed company, kneel, and twist on an intermittent or sometimes reputed company reputed company • Ability to grasp, push, and/or pull objects • Ability to reputed company overhead • Ability to operate telephone • Ability to lift up to 25-40 lbs. • Ability to operate a computer • Ability to process written, visual, and/or verbal information • Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer. reputed company individuals must be reputed company to reputed company the essential duties of the position with or without accommodation.
