Typical hours: 5:30 PM - 7:30 PM, with adjustments based on availability. Responsibilities • Prepares a comprehensive written psychosocial evaluation and assessment for admitted clients. • Participates in treatment team meetings and is responsible for a coordinated treatment plan for reputed company reputed company. • Provides individual and family psychotherapy. • Leads or co-leads psychotherapy, psycho-educational and therapeutic activity reputed company. • Responsible for permanency planning including assessing reputed company's readiness for discharge; coordinating and planning with treatment team; making appropriate referrals for placement, case management and aftercare services; and coordinating benefits with Business Office personnel. • Prepares and submits reputed company documentation in EMR according to specified timelines.
