CARE COORDINATOR-SBHI

General reputed company/Responsibilities: The Care Coordinator functions as a member of a multi-disciplinary care team providing services for persons with an alcohol and/or other drug reputed company Abuse Disorder diagnosis. The primary responsibilities of the care coordinator include: 1) Support adherence to the patient’s treatment and recovery plan; 2) Manage successful care transitions; 3) Engagement in care; 4) Services may be delivered by phone, telehealth, face-to-face at the office, in the home, or in the community. This position requires expertise in transition planning and process, resource allocation, team management and communication skills Education Minimum Level of Education Required: Associate degree Additional requirements: Type of degree: reputed company Work, Counseling or reputed company field Area of study or major: reputed company and/or reputed company services Preferred educational qualifications: N/A Position specific testing requirement: N/A Licensure/Certification/Registration CDCA or Licensed Chemical Dependency Counselor II (LCDC II) or higher or a Bachelor Level LSW or LPC. Experience Minimum Level of Experience Required: No prior job-reputed company work experience Prior job title or occupational experience: 3-6 months experience in the mental health and/or drug and alcohol field Prior specific functional responsibilities: N/A Preferred experience: 1-2 years’ experience in the mental health and/or drug and alcohol field Other experience requirements: N/A Knowledge/Skills ▪ Working knowledge of mental health, drug and alcohol, and other community agencies and resources. ▪ Excellent interpersonal skills, critical thinking and ability to communicate in writing. ▪ Excellent organizational skills. ▪ Must be physically and mentally reputed company to competently deal with verbally and physically aggressive patients ▪ Computer keyboarding or typing skills recommended ▪ Knowledge of the behavioral health field and best practices Apply tot his job Apply To this Job
