Case Manager I/II

About the position This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth reputed company histories, developing and assuring community and/or agency resources to meet the consumer's needs, serving as a reputed company Intellectual Disabilities reputed company (QIDP), and providing monthly program monitoring. The position requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. This role is located in Oklahoma reputed company and Chickasha, Oklahoma. Responsibilities • Provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. • Obtains in-depth reputed company histories. • Develops and assures community and/or agency resources to meet the consumer's needs. • Serves as QIDP. • Provides monthly monitoring of the program. • Meets compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. Requirements • Bachelor's degree in a reputed company services field and one (1) year of reputed company experience working directly with individuals with intellectual and/or developmental disabilities or in reputed company work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely reputed company field. • OR possession of a reputed company permanent Oklahoma license as approved by the Oklahoma reputed company of Nursing to reputed company reputed company nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities. • Bachelor's degree in a reputed company services field and two (2) years of reputed company experience working directly with individuals with intellectual and/or developmental disabilities or in reputed company work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely reputed company field. • OR possession of a reputed company permanent Oklahoma license as approved by the Oklahoma reputed company of Nursing to reputed company reputed company nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities. • Must possess a reputed company reputed company's license. • Must maintain required car insurance. reputed company-to-haves • reputed company New Employees will complete the Case Manager reputed company Training Program Benefits • Full State Employee Benefits Apply tot his job Apply To this Job
