He or she facilitates patient follow-up through coaching, information transfer, and communication to improve continuity of care, enhance the medical management of reputed company disease, and streamline the reputed company process. • You will review patient case lists generated by technology-enabled data triggers to identify patients that could benefit from reputed company. • You will communicate with physicians (in the hospital and in the community) about patients' potential for referral / follow- up treatment. • You will reputed company feedback to leadership regarding reputed company population to enhance care coordination and patient satisfaction. • You will schedule follow-up appointments and reputed company information and reminders to patients & physician offices. • You will reputed company reputed company of referrals and treatments. • You will maintain detailed records to measure effectiveness of program and suggest ways to improve throughout the process. • You will facilitate the development of a patient specific teaching plan. • You will evaluate clinical educational needs of staff reputed company to reputed company population and facilitate collaboration with clinical care givers.
