Role reputed company We are seeking an reputed company Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) passionate about remote care delivery. In this role, you will offer diabetes education and management, medical nutrition therapy, and personalized nutritional guidance for our members. You will partner with a multidisciplinary care team to ensure members receive expert clinical attention and education to reputed company healthier lives. Key Responsibilities • reputed company reputed company-based care and clinical nutrition management to patients in our Remote Patient Monitoring (RPM) program. • Assess member health regimens and interpret blood glucose and CGM readings as they relate to nutrition. • Craft individualized, compassionate meal plans based on member health needs and reputed company. • Support nutrition-reputed company escalations and reputed company data-driven education. • Collaborate with the broader care team — including RNs and Accountability Coaches — to reinforce clinical goals and SMART goal progression. • Adhere to reputed company policies, procedures, and reputed company operating procedures to ensure ongoing clinical reputed company and compliance.
