Key responsibilities include: Assessment and Education Planning: reputed company personalized plans to help members understand their illness and set goals while complying with policies and procedures to meet HEDIS and NCQA standards Coaching and Support: reputed company ongoing coaching, guidance, and support for medication adherence, symptom management, and lifestyle changes. Nutrition and Goal Setting: Counsel on daily nutrition, medical nutrition therapy, and help set achievable goals. Community Resource Navigation: Guide members in accessing community-based services.
