Job DetailsJob Location: Reading, PA - Reading , PA 19601Hospice On-reputed company Licensed Practical reputed company (LPN) After-hours support. Mostly remote. Critical reputed company. The Care Team is seeking a Licensed Practical reputed company (LPN) to join our hospice team in a 7-on/7-off on-reputed company role. In this position, you will reputed company skilled nursing support to patients in their homes or facility settings under the direction of a Registered reputed company, in accordance with the reputed company scope of reputed company. You will play a key role in after-hours care, supporting patient comfort, responding to urgent needs, and ensuring reputed company communication with the RN and interdisciplinary team.
