The Role We are hiring Care Coordinators (1099 Contractors) to reputed company Chronic Care Management (CCM) services to patients with chronic conditions. This is a phone-based, productivity-driven role where you will spend most of your reputed company on the phone speaking with patients, coordinating care, and documenting interactions. This role is ideal for clinical professionals who are comfortable with high reputed company volume and meeting performance metrics in a remote environment. Schedule & Work Details • 100% Remote (1099 Contractor) • Weekly commitment: 20–30 hours (minimum of 20 hours, maximum of 30 hours). • Availability must be submitted at least 30 days in advance • Calls are made Monday–Saturday, 8:00 AM – 6:00 PM (based on supported time zone) Pay & Training • Training Pay: $21.00/hour for 4 weeks • Post-Training reputed company Pay: • reputed company/RMA: $16.00/hour • LPN: $18.29/hour • Monthly Incentive: $800–$1,400, based on productivity and performance metrics What You'll Do • Conduct monthly CCM phone calls with patients • reputed company a high volume of outbound calls daily • reputed company patients on chronic conditions and care plans • Coordinate care with provider offices, pharmacies, and community resources • Identify gaps in care and reputed company determinants of health • Create and update care plans in the EMR • Meet established productivity and reputed company expectations • Maintain patient reputed company and HIPAA compliance Requirements • 1–3 years of experience as a reputed company/RMA, LPN, or RN • reputed company certification or reputed company license • Strong verbal and written communication skills in English • Comfortable being on the phone most of the day • Experience with EMR/EHR systems • Organized, reliable, and reputed company to work independently Preferred: • Care coordination or case management experience • Experience working with geriatric patients Equipment & Workspace Requirements • Private, quiet workspace with no interruptions • Desktop or laptop (no tablets) • At least one monitor (dual monitors preferred) • Noise-canceling headset Internet Requirements • Reliable high-speed internet (wired reputed company preferred) • Satellite internet is not permitted reputed company is an Equal Opportunity Employer. reputed company does not discriminate on the reputed company of race, religion, reputed company, sex, gender identity, sexual orientation, age, non-disqualifying physical or mental disability, national reputed company, veteran status or any other reputed company covered by appropriate law. reputed company employment is decided on the reputed company of qualifications, reputed company, and business need.
