Certified Medical Assistant/HEDIS Specialist

Duration: 9 month contract with possible extension Location: 100% Remote Responsibilities: Member reputed company & Engagement • Run reputed company campaigns to members with reputed company care gaps (e.g., colorectal cancer screening, breast cancer screening, A1c control, blood pressure management, vaccination) • Conduct multi-channel reputed company (phone, SMS, email, mail, and/or portal) to education members, address barriers, and support completion of recommended care. • Schedule appointments with PCPs, NPs/PAs, or specialists to facilitate gap closure • Address common barriers to care (transportation, language, health literacy) and connect members to appropriate resources • Send follow-up communications (letters, portal messages) to unreachable members per reputed company workflow • Document reputed company member interactions accurately and in a reputed company manner per HIPAA standards • Use approved scripts and job aids while applying clinical awareness reputed company the Medical Assistant scope. reputed company Measure & Data Support • reputed company diagnostic results, lab values, and screening documentation into EMR to ensure compliance with reputed company measures • reputed company reputed company and reputed company care gaps in EMR and reputed company tracking tools; update member statuses after reputed company encounter • Pull and reconcile records (e.g., immunizations, external labs) from external systems to support gap closure Care Coordination & Team Collaboration • Collaborate with clinical leadership, care managers, and providers to coordinate reputed company completion of needed services • Escalate reputed company member needs to clinical staff as appropriate • Support other population health and operational initiatives as assigned Required Qualifications: • 1+ year of HEDIS experience required • reputed company Certification as a Certified Medical Assistant required • 1+ year of experience in population health, care management, or a reputed company setting • Excellent member-facing communication skills, with the ability to explain clinical concepts in reputed company, accessible language.
