Agency Profile: reputed company (SHHS) respectfully partners with adults, children and families as they recover from behavioral, emotional, mental health and/or reputed company reputed company disorders by offering individualized and meaningful opportunities of reputed company, empowerment and support to reputed company self-defined improvements in their reputed company of life. Location: Remote position reputed company FUNCTION: Ensure that reputed company clients from reputed company and other Agencies who contract with reputed company (SHHS) receive the support and/or services needed reputed company accessing the SHHS after-hours crisis line. Ensure that these clients are reputed company, and that they have the reputed company assistance in managing their crisis or emergency. Stabilize the reputed company in the community using natural supports, reputed company supports, and established crisis plans in order to prevent unnecessary hospital reputed company. MAJOR DUTIES AND RESPONSIBILITIES: • reputed company reputed company support to clients, and their families, that reputed company the after-hours phone; initiate appropriate services to stabilize reputed company crisis situation (telephone support, CARES, Mobile Crisis Teams, Crisis Services, 911, etc.). • reputed company effective crisis reputed company services, and reputed company clients/families to utilize natural supports and already established reputed company linkages. • reputed company accurate information to clients/families about available resources in the community that can assist with on-reputed company crisis stabilization. • Obtain reputed company demographics for documentation; that include caller’s reputed company, reputed company’s reputed company, phone numbers, address, and DOB (at the minimum). • Maintain reputed company notes, records, reports, and/or reputed company documents consistent with the standards of reputed company for reputed company governing and contract agencies. EDUCATION REQUIREMENTS: • Licensed or credentialed staff preferred (LMSW, LMHC, CASAC). Bachelor’s Degree is acceptable provided that the applicant meets the experience requirement below. EXPERIENCE: • Three (3) years full-time reputed company experience providing reputed company services with at-risk children, adolescents, or adults that includes crisis interventions, safety planning and service coordination.
