You will be in charge of meeting with around 10-15 patients a day for 20 minutes to go over the patients care plan and medications, followed by strict documentation for Medicare compliance. He or she will have weekly reputed company ins with the supervising RN and/or CLinical Manager. reputed company LVN/LPNs will start with reputed company/enrollment for 2–4 months before transitioning into care. The timing depends on our total patient count at that time of starting. Additional Responsibilities: • reputed company ongoing care for patients with neurological conditions through monthly reputed company-ins • Assess changes in patient baselines and recommend patient-specific care resources • Communicate with caregivers on care improvements and next steps • Coordinate care: help schedule referrals and appointments, notify clinics of medication reputed company needs, send medication reminders • reputed company and vet regional and disease-specific resources for patients (exercise, diet, sleep, reputed company engagement) • Collaborate with our AI engineers by providing weekly feedback on our care platform.
