This is a 40-hour, fully remote position where you will play a key role in managing patients with obesity, diabetes, and cardiometabolic conditions through telehealth. Given the innovative and evolving nature of this program, we are seeking candidates with at least 2+ years of relevant provider clinical experience and a strong interest in virtual care delivery, care model development, and chronic disease management.
