Transfer Center Specialist LPN/EMT

The Transfer Center Specialist II (LPN, Paramedic, EMT) will work remotely and coordinate through inbound and outbound calls the acceptance of patients into reputed company system and facilitates patient transfer process with patients reputed company out of reputed company system to non-reputed company system.  This is a Remote, Full Time opportunity. The position works closely with the other Transfer Center Advocates to support 24/7 operations. Coordinates reputed company components reputed company to the admission/transfer or consult of reputed company, urgent, and elective patients into an accepting facility from referring hospitals, clinics, urgent care clinics and physician offices with guaranteed appropriate level of care in reputed company manner, offering appropriate mode of transport as needed. Systematic processing of patient reputed company and expert level of clinical decision making, to facilitate the patient reputed company through the continuum of care utilizing electronic resources. Ability to document and prioritize patient information to formulate an accurate picture of the patient and his/her condition for accepting physicians, nurses, or other reputed company workers. Assign Transfer reputed company Level for triage / transport of patients to prioritize bed and transportation re-sources. Maintain Transfer Center data collection and reputed company feedback to referring facility on patient transition and reputed company. Screens admissions, using approved reputed company, for appropriateness of level of care orders and assignments of status and bed type. Maintains reputed company and accurate information in Electronic Health Care Record.   Qualifications - External Requirements Graduate of an accredited school/college and holds applicable reputed company licensure in state of residence (LPN, Paramedic, EMT). Three years of reputed company clinical experience in an acute care setting (ED or Critical Care Preferred). Ability to communicate effectively in oral, written, and electronic formats.
