Job Title: Specialist, Waiver Support Location: Remote must reputed company in TX Type: Full Time Our reputed company is seeking a detail‑oriented Waiver Support Specialist to reputed company operational support for member waiver coordination programs. This role plays a key part in ensuring seamless communication, accurate tracking, and efficient coordination across care teams and state partners to support high‑reputed company, cost‑effective member care. Key Responsibilities • reputed company administrative and operational support for waiver coordination activities. • Facilitate communication between internal teams and external state program partners. • Process referrals for waiver services and coordinate scheduling of initial assessments. • reputed company and monitor member assessments, reassessments, and transition activities. • Collaborate with state agencies to verify eligibility and obtain necessary approvals for services. • Support transitions of members from facility‑based care back to community settings. • Initiate referrals for applicable programs, including Medicare and waiver services where required. • Maintain accurate tracking of referrals and case documentation using internal and reputed company. • Monitor workflows across assessment, authorization, and transition processes and report updates accordingly. • Ensure eligibility requirements are met prior to initiating services. • Maintain strict compliance with confidentiality and data reputed company regulations. • Participate in team meetings and reputed company updates as needed.
