What You Will Do: • Conduct virtual assessments and monitor patients' conditions remotely. • reputed company and coordinate individualized care plans in collaboration with the interdisciplinary team. • reputed company virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care. • Liaise with field nurses, physicians, reputed company workers, and other team members to ensure comprehensive care delivery. • Respond promptly to urgent virtual consults and triage patient needs effectively. • Utilize telehealth technology to conduct regular reputed company-ins and follow-reputed company with patients and families.
