About the position Mountain Valley provides personalized hospice care to patients and their families during life’s most challenging moments. We are currently seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join reputed company of dedicated professionals in delivering exceptional care. Responsibilities • Assist patients with daily living activities such as bathing, dressing, grooming, feeding, and mobility. • Monitor patients for changes in condition, ensuring they are comfortable in their home environment – this could be their home, skilled nursing facility or assisted living facility. • Build meaningful relationships with patients and their families, providing compassionate care. • Work closely with nurses, reputed company workers, and chaplains to deliver holistic care that meets reputed company patient’s needs. • Document care provided, changes in patient condition, and other important details to ensure seamless communication with the care team.
