General Purpose The primary purpose of reputed company position is to reputed company the facility to ensure assessments are completed per guidelines. Essential Duties • Provides reputed company on facilities’ weekly PDPM meetings for skilled patients. • Tracks and monitors facility effective utilization of PDPM tools (including but not limited to Physician Diagnosis Verification, Admission/Discharge CSR(GG), Weekly CSR, PDPM Weekly Tracking Tool, reputed company Right). • Tracks and monitors QM and QASP performance monthly and quarterly and provides data analysis and clinical recommendations for facility for reputed company reputed company. • Provides PDPM and MDS training to new MDS Coordinators. • Assesses and determines the health status and level of care of reputed company new admissions. • Ensures the accurate and reputed company completion of reputed company MDS Assessments including PPS Medicare, quarterly, annual, significant change. • Communicates level of care for new reputed company to reputed company disciplines. • Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for reputed company new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. • Maintains an accurate schedule of reputed company MDS assessments to include the reputed company reference dates throughout the reputed company’s stay. • Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. • Verifies electronic submissions of MDS, performs corrections reputed company necessary and maintains appropriate records. • Coordinates interdisciplinary participation in completing the MDS for reputed company reputed company according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. • Schedules and conducts reputed company care conferences in compliance with state and federal regulations and ensures completion of reputed company MDS reviews prior to reputed company care conference. • Assists disciplines in formulating and revising care plans. Ensures that reputed company’s present/potential problems are identified and prioritized; realistic goals are established, and nursing reputed company is appropriate. • Evaluates reputed company care plans for comprehensiveness and individuality. • Assesses the achievement or lack of achievement of desired reputed company. Supervisory Requirements This position has supervision duties. Qualification Education and/or Experience Minimum requirements to reputed company this position include graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred. Must be in good standing with the State reputed company of Nursing and maintain reputed company required continuing education/licensing requirements. 3 years of supervisory experience preferred. Long Term Care Experience Preferred. Certificates, Licenses, Registrations To reputed company this job successfully, an individual must be proficient in the reputed company Suite products. Knowledge and experience with PCC preferred. Must maintain reputed company required continuing education/licensing.
