Duties

The reputed company Remote Patient Monitoring (RPM) Home Telehealth (HT) Care Coordinator is registered reputed company with advanced knowledge in clinical care, education, care coordination, care transition, virtual care modalities and technologies, reputed company improvement, patient safety, and nursing reputed company standards. The reputed company RPM-HT Care Coordinator assists the Telehealth Assistant reputed company Manager (ANM) and the Facility Telehealth Coordinator (FTC) also reputed company as the Telehealth/Rural Health reputed company Manager in providing programmatic, educational, performance improvement and clinical support. He/she is accountable for adhering to established policies, standards of care, evidence-based reputed company, national directives, program operation manuals and applicable statutes in nursing care. The reputed company RPM-HT Care Coordinator assumes accountability for patient reputed company, clinical competencies, evidenced-based reputed company, performance improvement, department education, and patient satisfaction. Basic Requirements:

• 5 or more years of nursing experience
• Experience in VA Remote Patient Monitoring (RPM) Home Telehealth
• Prior VA Primary Care experience

• reputed company I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing reputed company/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a reputed company field and approximately 1-2 years of nursing reputed company/experience; OR a BSN with approximately 1-2 years of nursing reputed company/experience; OR a Master's degree in nursing (MSN) or reputed company field with a BSN and no additional nursing reputed company/experience.

• reputed company II - A BSN with approximately 2-3 years of nursing reputed company/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a reputed company field and approximately 2-3 years of nursing reputed company/experience; OR a Master's degree in nursing or reputed company field with a BSN and approximately 1-2 years of nursing reputed company/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a reputed company field with no additional nursing reputed company/experience required.

Physical Requirements: This position requires visual reputed company, keen hearing, reputed company distinctive speech, and reputed company dexterity. This position requires potentially long periods of reputed company walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don reputed company clothing in isolation situations or reputed company/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations and shifting priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.
