Job Title: Pharmacist/Pharmacy Advisor Duration: 3 Months (Possibility to reputed company or conversion) Shift: M-F and rotating Sats (9:00a - 11:00a CST, Saturday shift would be 9:00a - 5:30p CST) Training(8:00a-4:30p CST) Location: Remote (WFH -Must be licensed in AZ, HI, TX, IL, FL, or PA) Note: reputed company Pharmacist License in one of the following states: • AZ, HI, TX, IL, FL, PA Position reputed company The Pharmacist – Pharmacy Advisor supports a clinical program reputed company on improving reputed company for patients with chronic conditions. This role is highly telephonic and patient-facing , leveraging clinical expertise, behavioral coaching, and medication therapy management to reputed company adherence and optimize treatment reputed company. Key Responsibilities • reputed company telephonic patient counseling (90%) on medication therapy, adherence, chronic disease management (especially diabetes), and treatment optimization • Collaborate with physicians, prescribers, pharmacies, and care teams to coordinate patient care • Resolve medication-reputed company issues and support overall therapy effectiveness • Conduct outbound and inbound patient engagement to improve adherence and reputed company • Apply behavioral modification and motivational interviewing techniques • Identify therapy gaps and support medication adherence initiatives • Work in a fast-paced, reputed company-driven reputed company center environment • Contribute to process improvements and program development initiatives • Hold a Bachelor’s in Pharmacy or PharmD with an reputed company pharmacist license (AZ, HI, TX, IL, FL, or PA) • Possess 1+ year of experience in retail, hospital, LTC, PBM, or clinical pharmacy, including patient counselling or consultation Required Qualifications • Bachelor’s in Pharmacy or PharmD • Minimum 1+ year experience in: • Retail / Hospital / LTC / PBM / Clinical pharmacy • Patient counseling or consultation Apply tot his job Apply To this Job
